Caution: Canoeing and kayaking presents risks of injury or death.

 I acknowledge that canoeing and kayaking exposes me to conditions and risks of harm that may cause injury or death.  Conditions and risks of harm include, but are not limited to those caused by obstacles, contact with watercraft or persons, water conditions, weather and actions of others.

 I acknowledge that canoeing and kayaking is a physically strenuous activity.  I certify that I am physically fit to participate in canoeing and kayaking.

 I acknowledge that canoeing and kayaking  requires a certain level of skill.  I certify that I have the necessary skills to safely operate a canoe or kayak.

 I will wear a personal flotation device during the canoe or kayak trip. 

I hereby take action for myself, my executors, administrators, heirs, next of kin, successor and assigns as follows:

WAIVE, RELEASE AND DISCHARGE from any and all liability for my death,  disability, personal injury, property damage or actions of any kind which may hereafter accrue to me as a result of this canoe or kayak trip.

INDEMNIFY AND HOLD HARMLESS Wisconsin River Outfitters from any and all liability and expenses (including reasonable attorney’s fees) or claims made by other individuals and entities as a result of any of my actions during the canoe or kayak trip.

This form shall be construed broadly to provide a release and waiver to the maximum document; and I understand its contents.
